PHP Event Calendar

I would like to implement the following in PHP Event Calendar:
If the user click an Event, a small window popped up (with JavaScript?), and that based on the title of that specific event a form occur (in the small window) where the user is asked to fill in some information.

My questions: is this possible and how to implement this?
