Error Using calender.js

I am using calender.js on my jsp page as

<c:url var="calendar2" value="/pages/assets/js/calendar2.js" scope="request"/>
<script type="text/javascript" src="<rahul:out value="${calendar2}"/>">

<a href="javascript:cal1.popup();"><img src="<rahul:out value='${calendarpick}'/>" border="0" alt="Click Here to Select the Date"></a></td>

<script language="JavaScript">

var cal1 = new calendar2(document.forms[0].elements['requestDueDate']);
cal1.year_scroll = true;
cal1.time_comp = false;


When the page is loaded i get an error
"Error calling the calender : no target control specified"
Error:'' is null or not an object.

Its strange as the same code is working fine on other pages.

Please help me out.

and where is that document.forms[0].elements['requestDueDate'] that you refer to?
