calendar in existing form + how to set some days not selectable

Hello everybody,
Is it possible to have a copy of the modified script to use the calendar in an existing form?

In addition I have encountered the following two minor problems:
1) the bottons "prev" and "next" don't show up;
2) the second calendar in the form is without year scroll, while I'd like to have it with (like the 1st one). When I try to set the same kind of calendar (with year scroll) for both fields the selected date only shows up in the first field (even if the selection was made on the 2nd calendar, corresponding to the 2nd date field).

In addition I would like to know if it would be possible to set some date not selectable by the user on a calendar and, if so, how to do it.
