how can i modify the form(delete some items)

Hi I d/l the form here but I don't know how to delete some items like company and general info fields from the form. I hv suck with it for the whole day but still can't solve the problem
when i del the related fields from the table and del some javascript code that related to those fields, the form validation doesnt work since i updated it, the alert box didnt comes out once i click submit
pls help
